Wisconsin law
Wisconsin recognizes declarations to health care professionals (living wills) under Wis. Stat. § 154.01 et seq. and health care power of attorney under Wis. Stat. § 155.01 et seq. Both documents must be signed in the presence of two witnesses who are at least 18. A witness may not be related to the signer by blood, marriage or adoption, entitled to any part of the estate, directly financially responsible for the signer's health care, or a serving health care provider or covered employee. The health care agent may not witness the power of attorney. Witnesses may appear remotely by real-time audiovisual technology if the signing is supervised by a Wisconsin attorney and the other statutory conditions are met.
